Working with the physician, the physician’s staff, and the discharge counselors, Freedom Home Health develops a care plan with clear, measurable objectives. All services are ordered by and coordinated with the patient’s physician, and are delivered by our team of skilled health care professionals who are dedicated to helping each patient achieve the highest possible level of health and independence.

In a recent AARP study, 89% of seniors over 50 stated they would choose to age in place in their homes independently as long as possible. Freedom Senior Services provides support at all levels whether the client is at home or residing in a senior care community, an assisted living or long-term care community. Freedom Senior Services often provides services to assist with Activities of Daily Living (ADL’s) such as bathing, dressing, and grooming to help clients remain at home where they’re comfortable.
